LIFE-CHANGING COURSE STARTS IN SOUTHWARK

Co-creating Health is a new project aimed at helping patients with type 2 diabetes manage their condition better and take greater control of their lives.

The Trust's diabetes service and Southwark Primary Care Trust were granted funding from the Health Foundation, for this groundbreaking project to help improve diabetes services for people in Southwark.

The project started in early 2008 and includes short courses for patients and clinicians.

The unique thing about the programme is that patients and clinical staff are being trained to teach these courses together, as well as working together to identify improvements to the diabetes services in Southwark.

Medical staff are also being trained to support patients with 'self management' of their condition. In the first course, people were encouraged to set weekly goals for themselves throughout the six week course and to continue this in their everyday lives.

The aim is to empower patients to manage their condition through healthy eating, regular exercise and stress management.

At the end of the course the participants gave themselves an action plan and three month goals such as; as losing weight, continuing healthy eating, and going on holidays, which they may have been unable to do previously because of concerns about their health.

The Trust will be looking for more participants in the future.
